Certainly genuine. He used poor judgment in getting on the elevator when his bladder was that full, but that's typical of the age. I wonder if any of the others had to pee but weren't saying so.

Perhaps his intention is to take the elevator to some place where he can pee. I'm thinking of residential buildings where there's at least a toilet in each home, but none outside.

Can you tell us which part(s) of this film are worth watching, perhaps? It's 20 minutes long which is a lot to sit through.

Bodybuilders often drink large amounts of water during the day, so it's not surprising that one would need to pee urgently when driving anywhere.
