The poor guy is actually not able to pee because he can't bring himself to do it for psychological reasons. It's wrong to pee in a diaper, there are people around etc. So...this is basically pee shyness in action.
At 1:30 he actually says that his bladder is full to the brim but it doesn't work.
At 2:14 he says that he has a feeling that all the people around him know he's wearing a diaper and is wetting said diaper.
At 2:25 he talks about how comfortable it is (supposed to be) to wear a diaper because you don't have to worry about bathrooms and that it's actually what everyone wants but it's still not working for him. He then says that he's about to lose his mind and tries to calm down a little.
At 2:55 he is very upset and doesn't want to continue. The woman teases him, saying that it can't possibly be so hard to pee and he snaps at her and tells her that it isn't funny anymore because he's bursting and has been on the move for the whole day.
At 3:33 he manages to pee a little bit and he's happy about it, but he also mentions that it was really hard for him to do and that he has a lot of respect for everyone who's incontinent because it's not a nice feeling to pee yourself in public.
